Office of Financial Aid Data Request Form
If you are a student or parent, do not complete this form. Instead, contact a representative from the Office of Financial Aid using one of the methods listed above.
Statutory and regulatory restrictions govern the sharing of student financial aid information. CSU offices and departments, and outside entities may request student-level financial aid data to better serve the institution’s students and monitor student outcomes. When making a request, the Office of Financial Aid must determine what information can be provided without violating the Family Educational Rights and Privacy Act (FERPA), the Higher Education Act of 1965 (HEA),or the Privacy Act restrictions.
Data requests must be made in writing with specific details to ensure that all parties understand what data is being requested and how the data will be used. Requests may be denied or modified to ensure compliance with applicable laws and to safeguard students’ privacy rights. Please note these laws, along with institutional policies, are subject to change annually. Therefore, the ability to provide certain data in the past does not guarantee the same data can be provided for current or future requests.
